Admissions
The Plant Home is a tax-credit senior apartment complex offering assisted living services. We verify the income and assets of all our residents to assure they qualify under the IRS income guidelines. All residents must also qualify for assisted living care. This page provides prospective residents with information ranging from income guidelines and financial assistance policies, to applications for residency. Criteria for Admission
In order to be considered eligible for services offered by PALS residents must:
Be 62 years of age or older.
Have at least one ADL need and/or 2 IADLs
ADL (Activities of Daily Living) needs are classified as but not limited to:
- Bed mobility: How a person moves to or from a lying position, turns side to
side, and positions his/her body while in bed.
- Transfer: How a person moves between surfaces to/from bed, wheelchair,
standing position (excluding to/from bath/toilet.)
- Locomotion: How a person moves between locations, in room and other
areas. If a wheelchair, self-sufficiency once in chair.
- Eating: How a person eats and drinks (regardless of skill)
- Toilet Use: How a person uses the toilet room (commode, bedpan, urinal)
Transfers on/off toilet, cleanses, changes pad, manages ostomy or catheter,
and adjusts clothing.
- Bathing: How a person takes full body bath/shower, sponge bath and
transfers in/out of tub/shower. (excludes washing of back and hair)
- Dressing: How a person puts on fastens, and takes off all items or street
clothing, including donning and removing prosthesis.
Have a need for at least two (2) IADLs
IADL (Instrumental Activities of Daily Living) are classified as but not limited to:
- Meal preparation: Prepared breakfast and/or light meals.
- Preparation and receipt of main meal
- Telephone usage: able to contact people in an emergency
- Routine housework: vacuuming, dishes, dusting, making own bed, trash
removal, cleaning floors, and cleaning bathroom as needed.
- Managing Finances: Managed own finances, including banking, handling
checkbook, and paying bills.
- Grocery Shopping: Did grocery shopping as needed including the storage of
the items (excluding the transportation)
- Laundry: either within or using an outside laundry facility
- Transportation: Necessary to perform covered services described in residents
Service Plan.
Be able to transfer with no more than 1 assist and move along on a flat surface
using a walker, cane, or wheelchair in order to evacuate the building.
Be able to dress, bath, groom, and toilet independently or with no more than one
assist.
Require no assistance with meals or only minimal assistance with meals.
Assistance includes cueing, giving direction, cutting up food and supervision.
Cueing means any spoken instruction or physical guidance, which serves as a
signal to do something.
Have no communicable disease requiring isolation.
Be able to evacuate the building with minimal assistance.
Be free from acute episodes of psychiatric illness not managed by appropriate
medical treatment.
